Repeat-switch deletions

Upload the CSV from docs/repeat-switches-deletions-extract.sql — hard-deleted-upstream household counts (per cohort month + first/repeat) that rebase the cohort waterfall's Total. Re-uploading is idempotent (last write wins per month + ordinal).
